An organised strategy for managing long-term illnesses like diabetes, heart disease, and asthma is known as a Chronic Care Management (CCM) program. The program offers patients continuous support and direction, including care coordination, medication management, and frequent phone or video check-ins. Individualised treatment planning, goal-setting, and self-management education are provided to patients. The program seeks to improve overall health outcomes, decrease hospitalisations and ED visits, and give people the tools they need to take charge of their health.
